Social Prescribers

Social Prescribing – What is it and how can it help me?

Many things affect our health and wellbeing and may people are troubled by things that can’t be cured by medication or medical treatments. Things such as loneliness, isolation, emotional wellbeing, bereavement, stress, being a carer, debt, poor housing, and the practicalities of managing long-term conditions all have an effect on our well-being.  Many people feel overwhelmed and can’t reach out to make connections that could make a difference to them.

This is where the Social Prescribing team comes in …  we support people with a range of social, emotional and practical needs.  First of all, we will listen to what matters to you as an individual and then connect you to activities, community groups, organisations and agencies that can make a really positive difference to your wellbeing.  We will meet with you or talk to you regularly over the following months to see how you’re getting on and to offer further support if needed.

There are Social Prescribers attached to each of the six GP surgeries in the Bideford, Torrington and Hartland area.  Just contact one of our team;
